Do I have to pay for Kidslox for each device?

No, you only need to pay for Kidslox on a per-account basis.

On Family plan you can add up to 10 child devices and unlimited number of parent devices with no additional cost.

On the other hand, Basic plan supervises only one child's device with a limited feature set.
